This form is to be used by students who are completing or have recently completed Australian approved programs of study in 2016/2017 and who wish to apply for registration as a health practitioner in Australia or provisional psychologist applicants who have completed their study within the last 10 years.

Who should complete this form?

The following people should complete this form:

  • Have recently completed, or are scheduled to complete, an approved program of study in 2016/2017.
  • Have never been registered as a health practitioner in Australia or overseas.
  • Have never practised as a health practitioner in Australia or overseas.

Who should not complete this form?

Do not complete this form if:

  • You have been previously registered as a health practitioner in Australia or overseas (e.g. an applicant for medical practitioner registration who has previously been registered as a physiotherapist).
  • You have practised as a health practitioner in Australia or overseas.
  • You have completed a program of study or qualification overseas.
  • You are applying for registration in more than one profession (e.g. nurse and midwife) or more than one division of a profession (e.g. dental therapist and dental hygienist).
  • You wish to pay by cheque, money order, or in person. 
  • You are an enrolled nurse and wish to apply for registration as a registered nurse or midwife.

Before you start

Are you applying four to six weeks before completing your course? The disclosures you are about to make must be as recent as possible when your application is assessed. If you apply too early your application may not be accepted.

Before starting your application please ensure that you have the following:

  • A Visa or MasterCard credit or debit card to pay the application and registration fees using our secure payment gateway.
  • Details of your qualification including your student identification number.
  • You may be required to obtain an international criminal history check (ICHC) report from an approved vendor. An ICHC report is required if you: 
    • Declare a criminal history outside Australia, and/or 
    • Have lived, or been primarily based in one or more countries other than Australia for six consecutive months or longer, when aged 18 years or more.

Allow 10-15 minutes to complete and submit this online application. You will not be able to save details entered and come back at a later time.

Graduate online application process

  1. Complete this online application.
  2. Provide documents to AHPRA within 30 days. 
  3. Your university will notify AHPRA when you are eligible to graduate.
  4. AHPRA processes your application for registration.
  5. If your application is successful, AHPRA will send you a registration certificate and list your details on the Public Register.
